What to Expect at Your First Visit

​We will take a complete and thorough history and provide a space for you to voice your concerns. We will work together with you to decide how to proceed, with complete respect for YOUR health care choices. We will send you home with a treatment plan the day of your visit to get you started on your health goals right away!

Appointments

For your convenience, we offer the following methods to schedule your appointments:

  • Online scheduling: To request appointment availability, please click here to fill out the form. Our scheduling coordinator will contact you to confirm your appointment.
  • Call our office at 623-556-7130.
  • Email us at frontdesk@youngagainaz.com with your preferred date or day of the week and time range, and our scheduling coordinator will contact you to confirm your appointment.

Please call our office at least 24 hours in advance of your appointment. We’ll work with you to help you find another date that fits your schedule.

We understand that unforeseen situations may arise that are out of your control. 

You will get a reminder text, email, or call 24 hours prior to your appointment. Patients who do not cancel their appointment within 24 hours will be charged a $50.00 cancellation fee.

We will conduct a thorough examination on your first visit to determine the best course of action to achieve your weight loss goals. We will:

  • Get an idea of your diet and lifestyle  
  • Go over your medical history
  • Discuss a diet plan that works for you
  • Give you a B12 injection in office
  • Send you home with medication or supplements  
  • Possibly give you a lab order  

Treatments

Payments for services at Young Again Wellness & Weight Loss Clinic are due at the time services are rendered.

We offer these payment options:

  • Cash, check or credit card at time of service
  • HSA or FSA cards

Payments for services at Young Again Wellness & Weight Loss Clinic are due at the time services are rendered. Our goal is to offer continuing, optimum patient care without having patients’ treatment dictated by a third party; therefore, we do not accept payment from insurance companies. Instead, we prefer to have a direct relationship with our patients and to give them the opportunity to make the final decision as to which treatments are most beneficial for them.

We can help with insurance reimbursement claims. The majority of insurance companies consider Naturopathic Physicians as out-of-network providers.

​Filing claims with insurance companies is a full-time job that requires a substantial amount of knowledge that Insurance Coders go to school to learn. Our staff has not been trained for this purpose; however, we do our best to comply with insurance requirements so that you can get reimbursement. Let us remind you that submission of a claim for reimbursement does not guarantee reimbursement and is subject to health plan benefits. To help you in your quest for reimbursement we will do the following:

  • Provide you with a super bill that summarizes the services rendered, diagnoses and charges applied
  • Gladly fulfill any requests for additional forms from your insurance company
  • Give the copied information to you to submit to your insurance company (along with their letter of request). We HIGHLY recommend that you only send copies of the documents, never the originals


Please note that if we receive letters from insurance companies requesting records or different diagnosis codes, different CPT codes and additional information we will give this additional information to you to submit to your insurance company; however, we will not send the information directly to the insurance company.

We ask that all patients complete our patient registration form and read and sign our financial policy prior to seeing the doctor(s).

Any payment by insurance should be sent directly to you, the patient. Call your insurance company to request claim forms to submit for reimbursement. 

Unfortunately, we do not accept CareCredit at this time.
